How should I prepare for my first appointment?

Once you have made an appointment, please print and fill out our Patient Medication and Supplement Form and our Additional Insurance Information Form.

In addition to these two forms, please remember to bring:

  • Loose fitting, comfortable clothing to wear for your assessment/treatment (we suggest shorts and a tank top or t-shirt)

  • reading glasses (if necessary)

  • any relevant medical reports, x-rays. MRI's, etc

  • health card or other provincial/federal insurance information contact information of you family physician

Our office is a scent-free environment, please respect this by avoiding perfume or scented products.

Are services at the clinic covered by OHIP?

Dr. Rapson can bill OHIP for services that are covered by OHIP such as assessments, counselling and virtual consultations.

As mentioned on our services & fees page, OHIP does not cover acupuncture, osteopathy, physiotherapy or Traditional Chinese Medicine assessment and treatments. These are considered 'non-insured services'. Fees for all ‘non-insured services’ are the responsibility of the patient or their guardian.

What payment methods do you accept?

We accept e-transfer, cash, cheque, VISA, Master Card or debit card. We do not accept American Express. The clinic’s e-transfer is set up with automatic deposit, so there is no password. To make a payment to us via e-transfer just add our email (rapsonpainclinic@gmail.com) to your list of recipients and send the payment.

Do you accept people who are victims of motor vehicle accidents?

Yes we do. We are a FSCO licensed facility. We treat individuals with injuries from motor vehicle accidents. We submit OCF-18 treatment plans for approval by insurers. If the plan is rejected by the insurer, we charge a fee to the patient. If it is accepted, the insurer pays us for preparing the OCF-18 and for the treatments.

Do you accept WSIB patients?

Regrettably, we no longer accept new WSIB patients.

Is there parking nearby the clinic?

Our clinic is Suite 207 within The Rosedale Medical Centre at 600 Sherbourne Street, across the street from the Sherbourne subway station on Line 2 (the East/West line). The Sherbourne subway is wheelchair accessible. The Rosedale Medical Centre has a small pay-parking lot behind the building. You can access it from Selby Street which goes one way west from Sherbourne Street one block south of Bloor Street. Alternatively, Greenwin Square has public, underground parking which can be accessed from Huntley Street, one block west of Sherbourne Street.

There is very limited street parking on Sherbourne Street or Selby Street but some patients park north of the bridge into Rosedale and walk south to the clinic. We recommend taking public transit whenever possible as pay-parking can be costly and in high demand.
