Important Notice: Rapson Pain Clinic Ownership has changed

Dear Patients of Rapson Pain Clinic (RPC)

This message is sent to you with sincere appreciation for your having given us the privilege of taking part in your health care in the past.

We have news of changes to RPC. As of March 31st Irene Biemann, Rosemary Anderson, Odi Acosta and I will be leaving RPC.

Danny Li and Wendy Wei Zhang now have ownership of the clinic and will continue their Traditional Chinese Medicine (TCM) and Acupuncture practices there. Both of them are Registered TCM Practitioners and Acupuncturists (RTCMP and RAc).

Under the College of Physicians and Surgeons of Ontario (CPSO) Medical Records Management Policy, as the current custodial physician of your medical records at the clinic, I must retain your original clinic medical records in a secure manner for 10 years after your last clinic visit. The records will be stored securely with no public access. After 10 years the original charts will be shredded. I may only transfer copies of your medical records or a summary of your chart to another practitioner with your written consent. There will be a per page fee for the copying.

Please complete, sign and return to us the linked form indicating that you understand what will happen with your clinic medical chart.

We would appreciate it if you would return the attached form to us by February 15, 2024. Please note that even if you wish to continue being treated at RPC, a formal transfer of your chart must be made, so please fill in the section of the form for sending a copy of it to a new practitioner. In that case, of course, there will be no courier fee charged; however, the photocopying fee will need to be paid. Should you need some guidance on transferring your care and your medical chart, in the near future we will have a list of suggested practitioners available at reception.
